What do I do if a name is misspelled or incorrect data appears on my marriage license?

In order to correct information on a previously issued marriage license, both parties will need to appear in person at either of our locations to request any necessary corrections.

Both parties must bring with them Valid Photo Id's, Original Birth Certificates and any other supporting documentation as to the necessary correction.

The fee to amend a marriage license is $30 plus $10 for recording the required "Affidavit of Correction", If additional affidavit's are needed (witnesses) $8.50 per additional page will apply to the fee.

If you live out of the state of Florida we can email you forms to make corrections. The forms must be notarized and accompanied by your payments via money order or certified funds for $30 and $10 or $18.50. (Please submit separate payments). By mail you must provide copies of valid photo ID for each party; Original Birth Certificates and any other supporting documentation as to the necessary correction.  These are not completed in person and will be returned to you upon completion. 

Please include a self-addressed stamped envelope for the return of your certified amended marriage license and original documents.
