If an order referring parties to mediation has been issued in a family law case, this form would be used to request that mediation be waived for good cause and that any prepaid mediation fees be refunded to the party or parties. Once the petitioning party receives an order back from the Judge granting mediation fees be refunded, then he/she would mail a copy of the order along with the application for refund.
